Adult Blood Test Appointments (Age 18 and over)

If you need to book a blood test – we are able to offer this service to our patients. Please ensure you’ve spoken to a GP/Nurse and obtained and ICE form before booking your appointment.

This service is for patients registered within Oxfordshire or receiving care within OUH. Please ensure you have a doctor’s order for your blood test. If the blood order is from your GP, please bring the A4 blood request form containing labels (ICE form).

Please book your blood test at least one week before your hospital appointment.

Call 01865 222652 if: 

• Your blood test is needed in the next 3 days and there are no slots available
• You need to cancel or reschedule your blood test

The Oxford Community Diagnostic Centre is located inside the Perspectum Building, Gemini One, 5520 John Smith Drive, Oxford, OX4 2LL.

There is free parking adjacent to the building. There are 2 nearby bus stops, they are located on Garsington Road, and Barns Road. Please contact your local Bus Service provider for details on times and stops.
