PATIENT RIGHTS
AND RESPONSIBILITIES
We will do our best to look after
you;
If your problem is urgent, you will
be seen on the same day.
You may see any of the doctors or
nurses, but please ask to see the
same person throughout one illness.
If you arrive late for an
appointment, you may be asked to
wait until the end of the surgery to
be seen so that disruption to other
patients is minimised at the
discretion of the Doctor/Nurse. You
may be asked to book another
appointment.
If you cannot keep an appointment
please let us know so that it can be
made available for someone else.
You have a right to see your medical
record (see practice manager for
details). We will ensure the
contents of your records are
accessible to healthcare
professionals who need to know and
ensure absolute confidentiality.
Our staff have the right to care for
others without being attacked or
abused.
We follow a policy of zero tolerance
and are committed to do everything
possible to protect all our staff,
patients and visitors from
unacceptable behaviour.
Severe or persistent cases of
violence (verbal or physical) will
result in removal from our practice
patient list.
Identifiable patient information
will only be shared within the
practice team and, in the case of
referral, to the clinician to whom
the patient is referred.
Disclosure of identifiable patient
information to any other outside
agencies will only be done after
receiving written permission from
the patient.
The freedom of information act 2000
obliges us to produce a publication
scheme, which is a guide to the
classes of information the practice
intends to make routinely available.
Copies are available from the
Practice Manager.
