All new patients
registering with the practice will
be asked to complete a medical
questionnaire and registration form.
You will be asked for proof of
address (utility bill, bank
statement etc) and proof of ID
(driving licence, passport) etc.
You may indicate on your
registration form any preference for
a particular doctor within the
practice.
